Cigarette, Tobacco, and Electronic Vaping License Application Update
The Cigarette, Tobacco, and Electronic Vaping Device Retail License Application (CTV-100) has been revised as of February 2026.
It includes an attestation regarding compliance with Wisconsin's Electronic Vaping Device Directory
Applicants should use this revised form to apply for new and renewed licenses
Alcohol Beverage License Application Forms are Updated
Forms AB-100, AB-101, and AB-200 have comprehensive instructions preceding the form. For additional information about how to use the forms, see the instructions.
Forms AB-100 and AB-101 have updated instructions. No changes were made to the contents of the forms.
Form AB-200 has been revised to allow for a more streamlined renewal process that does not require licensees to provide new Forms AB-100 and AB-101 each year if the paperwork from the previous license year is still accurate
Form AB-200 now includes Appendix A which creates a standardized listing of all persons involved in the applicant business and an easy way to identify when personal information has changed.
Right-of-Way (ROW) Permit Application - *also used for placement of Dumpsters, Mobile Storage, Trailers, and Construction/Demolition Units in the public right-of-way.
7.4. Exempted signs. The Village allows noncommercial messages on any permitted sign. In addition, the Village exempts the following types of signs from the requirements of this ordinance:
Political signs that are displayed not more than four weeks prior to the governmental election to which they apply, and removed within seven days following such election.
