 Cigarette, Tobacco, and Electronic Vaping License Application Update

The Cigarette, Tobacco, and Electronic Vaping Device Retail License Application (CTV-100) has been revised as of February 2026.

• It includes an attestation regarding compliance with Wisconsin's Electronic Vaping Device Directory
• Applicants should use this revised form to apply for new and renewed licenses

Alcohol Beverage License Application Forms are Updated

Forms AB-100, AB-101, and AB-200 have comprehensive instructions preceding the form. For additional information about how to use the forms, see the instructions.

• Forms AB-100 and AB-101 have updated instructions. No changes were made to the contents of the forms.

• Form AB-200 has been revised to allow for a more streamlined renewal process that does not require licensees to provide new Forms AB-100 and AB-101 each year if the paperwork from the previous license year is still accurate
• Form AB-200 now includes Appendix A which creates a standardized listing of all persons involved in the applicant business and an easy way to identify when personal information has changed.
